Self-Certification Form

All members on the Access Plan must complete self-certification. This happens during new membership applications or existing members can complete the form below. Submission of this form does not enroll you in the Access Plan.

Access Plan Qualifications

To qualify for the Access Plan, the member must self-certify that their household income is 50% or less of the Area Median Income. 

No paperwork or financial statements are required to be on the plan; the self-certification selection was designed to avoid many of the hassles normally associated with income-based programs. If you meet the qualifications, your registration fee is reduced to $1; select this option during your application. Registration fees are non-refundable.
