Title VI Complaint form

This Title VI Complaint Procedure is established to meet the requirements of Title VI of the Civil Rights Act of 1964, as amended. Title VI prohibits discrimination on the basis of race, color, or national origin in programs and activities receiving federal financial assistance. HOURCAR is committed to ensuring that no person is excluded from participation in, denied the benefits of, or otherwise subjected to discrimination under any HOURCAR program or activity on the basis of race, color, or national origin.

The Title VI mandates that individuals with disabilities shall not be excluded from participation in, denied the benefit of, or be subjected to discrimination under any program or activity. This Title VI Complaint procedure can be used by anyone who wishes to file a complaint alleging discrimination on the basis of disability in the provision of services, activities, programs, or benefits by HOURCAR.

HOURCAR takes these complaints seriously. It may take up to 30 days to investigate the complaint and provide its determination.

Process for filing a Title VI Complaint

The relevant departments will be notified of the complaint and will work and will work with the complainant to resolve the issue. A response will be shared with the complainant and will explain the position of HOURCAR and offer options for substantive resolution of the complaint. If the response by HOURCAR does not satisfactorily resolve the issue, the complainant may appeal the decision. HOURCAR will meet with the complainant to discuss the complaint and possible resolutions before responding with a final resolution.
